You will need to complete a "Request for Counseling" form to enable SCORE Counselors to understand your business and the assistance you need. The form also provides information to contact you so we can arrange an appointment for counseling / mentoring. 
Instructions to complete "Request for Counseling" form
► IMPORTANT - Please read carefully before clicking the link below
1) When you go to the Log-On Page (link below), select: New Client - Register Here. This quickly takes you to SCORE's "Request for Counseling" form.
Note - The form will open in a separate browser window, so you can always refer to these instructions.
2) Fill in all the required fields on the form. If you do not have an email address, enter: na@na.com
3) In the text box at the center of the form, marked "describe specific assistance requested."

♦ Tell us the Issues for which you request assistance.
♦ Include your Website (if you have one).
This information is important for SCORE counselors to understand what you need.
4) After completing all the required fields, click Submit at the bottom of the form.
Additional Information - After submitting the form, exit the window and you will return to this page. Here are some Tips for your First Counseling Session:
► Our office will phone you within one or two days to schedule an appointment.
The call will be during our office hours: 9:00am - 1:00pm, Monday through Friday.

► The first session works best when you bring examples of what you want to discuss:
Your Questions, Product Samples, Brochures, Financial Statements, etc.
► You will have two very experienced SCORE Counselors offering you guidance with your business decisions. Their time, reserved for you, is valuable, so if you need to cancel or change your appointment, please notify us at least 24 hours in advance.
